We’re Here for Your Home Health Care and Hospice Needs

Home health and hospice services continue to be critical at this time. We are working during these unprecedented times to ensure you (or your loved one) have access to the services that have been ordered for you (or them).
• We are staying up-to-date with Medicare regulation changes to serve patients more easily.
• We are following and implementing all recommended CDC guidance for infection control measures and appropriate use of personal protective equipment (PPE). Clinicians are wearing gowns, masks, goggles, and gloves for ALL patient care. All of our clinicians have been checked off on PPE competency and continue to complete online COVID-19 training courses as new information becomes available.
• We are limiting equipment being brought into the home to only essential items necessary to perform treatments.
• We are coordinating closely with the state and local health departments.
• We are screening all of our clinicians daily, including monitoring temperatures, any travel, symptoms, and or potential exposure to COVID-19. Clinicians must report immediately and self-quarantine for at least 14 days if they have any symptoms or exposure.
• We evaluate and track all current and prospective patients for COVID-19 risks before providing care, and continue to track prior to and during each visit.

While we share the great concern about this pandemic, we feel confident in our ability to provide uninterrupted care in a way that will minimize exposure to others. We are prepared to continue to provide exceptional care.

Your primary care physician will determine if you have a medically necessary home health care need. Your primary care physician can fax your referral to our Intake Department at 262-923-1213. If you have questions about home health (skilled nursing or therapy) or hospice please call us at 262-293-3951.

We at Preceptor Home Health and Hospice have COVID-19 prevention best practice procedures in place so that you have peace of mind as you begin or continue service with us. We know you share our concerns about this issue and want to assure you that we are working diligently to ensure all patients and staff in our community can be as safe and healthy as possible.

Here are some steps we are taking:
– We are staying up-to-date and implementing CDC guidance, including CDC’s March 10 guidance relating to home health and hospice providers.
– We are following recommended infection control measures, including hand hygiene, environmental cleaning, and use of personal protective equipment (PPE)
– We are evaluating all patients and prospective patients for COVID-19 symptoms or risk before providing care
– We are screening, monitoring and tracking the health of our employees, including repeated screening for travel, symptoms, or potential exposure
– Where necessary, we are coordinating closely with state and local health departments

As a result of our efforts, while we share the community’s great concern about this pandemic, we feel confident in our ability to provide uninterrupted care in a way that will minimize exposure to others. We hope that you will use us as a resource to help prevent the spread of COVID-19.
