What is Hospice?
Hospice is a specialized type of care for those facing a life limiting illness that gives them peace, comfort, and dignity. It also provides programs to families that support them after their loved one passes away.
Hospice care focuses on the quality of life rather than its length. It provides humane and compassionate care for people during their final journey so that they may live as fully and comfortably as possible.
The hospice philosophy accepts death as the final stage of life; it affirms life and does not hasten or postpone death. Hospice care treats the person rather than the disease, working to manage symptoms so that a person’s last days may be spent with dignity, surrounded by people who care.
Preceptor Hospice Care empowers our patients and their families by directing their end of life care through each stage by focusing on their individualized goals. We support our patients wherever they call home, whether that is a private home, assisted living center, nursing home, or group home.
Together we work with the patient, their family, caregivers, primary care doctor, and our team which includes a hospice medical director, nurses, social workers, certified nursing assistants, volunteers, dieticians, music therapists, massage therapists, and chaplains to develop an individualized care plan that focuses on a holistic approach.
Eligibility
Patients are eligible for hospice care when a physician makes a clinical determination that life expectancy is 6 months or less if the terminal illness runs its normal course. For more details regarding eligibility please call our office at (262) 735-4297.
Payment Options
Eligible patients have coverage for hospice care under Medicare, most state Medicaid plans and many private insurance plans, from routine care to grief support services. Hospice Care also covers any equipment or medications that are related to the terminal diagnosis of the patient. Volunteers
Volunteers are an important part of the Hospice team. Our volunteers provide support to our patients and their families. They also provide clerical support to our office staff. Hospice volunteers give comfort, peace, and care to patients, caregivers, and families during end of life. Our volunteers are an integral part of our hospice program. Philosophy
