Medicare, Medicaid, and most private insurances cover hospice care. Serenity Hospice Care covers any equipment and medications that are related to the terminal diagnosis of the patient.
The hospice nurse at the time of admission will develop a comprehensive plan of care which will address the specific needs of the patient. Visit frequencies may vary as a patient’s illness progresses. Serenity Hospice Care will have the nurse case manager visit at least once a week. The patient and/or loved one may decline visits from other team members.
Serenity Hospice Care is available 24 hours a day, 7 days a week for patient care. We encourage the patient, loved one, and caregiver to call any time. Our hours of operation are 7:30 AM to 4:00 PM. After business hours you will reach a specially trained nurse to handle your questions or concerns. At times the situation can be handled on the telephone; at other times a visit may be required by a team member. We encourage patient and loved ones to call the office before calling 911 unless you want life prolonging, aggressive treatment.
The hospice team will continue to assess the patient’s needs on an ongoing basis. They will make any necessary recommendations as needed for the benefit of both the patient and/or caregiver.
Serenity Hospice Care remains active in the patient care during the terminal illness. When a patient passes away, the loved ones are transferred to our bereavement program for additional support.
A patient can cancel or revoke the hospice benefit of service at any time for any reason. At times hospice may discharge a patient for different circumstances such as the patient’s condition improves, a patient and/or loved one is not willing to comply with the plan of care, the patient moves out of the designated service area, and/or there are safety issues that cannot be resolved. If a patient no longer needs the service and is discharged, they may resume at a later date as needed.
